A 13 year-old male was known to have had hypertension for 16 months. This had gradually progressed, despite medical therapy. On physical examination revealed a blood pressure of 260/220 mmHg, and there were grade 4 retinopathy. An intravenous pyelogram showed that the right kidney was much smaller than the left. Aortography demonstrated narrowing of the right main renal artery. Following right nephrectomy, the blood pressure remained at about 170-150/110-100 mmHg. With antihypertensive drugs for over a year since surgery. Study of the excised right kidney revealed an abrupt narrowing of the main artery caused by medial muscular thickening.
